WVSA State Championship
The spring calendar is very busy in May and June, and we have tried to schedule dates to allow teams to participate in the Spring Cups and still allow players to participate in the ODP programs.
There are NO PRELIMINARY ROUNDS
Region I drop date is May 30,2004. Any WVSA team that drops from the competition after that date, leaving a bracket without any entries, will be required to pay the $1500 fine assessed on WVSA by Region I.
Upon receipt of your entry (which must include the entry fee), you will receive written confirmation. You will be sent the Rules and Regulations of the competition along with the forms that will be needed to compete. Be certain that you have followed the regulations concerning team and player eligibility (found in the WVSA Manual). You must submit a WVSA State Championship state stamped roster, valid USYS pass cards, coaches cards, Coaches Code of Conduct form, Team Information Sheet, and Medical Release Forms. All of these forms are in this packet and can be obtained from the WVSA Web site, or from the Fax-on-demand.
The championship weekend will be held May 15-16 at the YMCA Youth Sports Complex in Beckley,WV

Forms required before the first game in Beckley,WV May 15-16,2004:
Note: Players are not required to sign in, all of the forms below can be presented by the Coach or Manager.
Player Pass Cards: all players must have valid player pass cards, including players not in attendance. Cards should be laminated.
Coaches Pass Cards: all members of the coaching staff (4 persons max per team) must have a valid coaches pass. Persons without a coaching pass will not be allowed on the team side
Medical Releases: This packet contains a copy of the US Youth Soccer Medical release. If you have medical releases for your players through their registration, you need not use this form. However, all players must have a Medical Release presented at the time of Check-in prior to your first game.
NO Medical Release= NO Play NO EXCEPTIONS
Coaches Code of Conduct: All members of a teams coaching staff must sign the Code of Conduct.
State Stamped Roster: All teams must present an original State Stamped WVSA State Championship roster prior to their first game. The roster is obtained from the WVSA State office and must be sent or faxed to the State Office by May 6. Remember that May 6 is the date that all rosters are frozen.
Team Information Sheet: This form allows us to contact you during the competition and inform you of any changes in schedule, game times, etc.
Region I Housing Form: This form needs to be completed before your first game in Wheeling. It is a form that documents what rooms will be required for your team at the Region I Championships at Providence, Rhode Island should you qualify. The form will be returned to you or destroyed if you do not advance. This form will be sent to you separately by the Cup Housing Coordinator,Theresa McCrady and should be completed electronically and submitted on disk at registration.
Forms required before Championship Games in Beckley:
Blue Roster: The Blue Roster is a Word document that is available from the Region I web site or the WVSA . The Blue Roster will be used for team check-in before the Championship Game. The form should be completed electronically and submitted on Disk/CD at registration. Make sure you include uniform numbers on both the primary and alternate jerseys. The Cup Committee will check and make any necessary corrections and then print the form for Team Check- In prior to the Championship Game.
